To complete your application for the HBOT4Heroes program, please follow the steps below.
If you’re unable to finish the process now, don’t worry—a confirmation email with instructions and the required form has been sent to your inbox. Our Veteran Treatment Coordinator will also follow up with a phone call soon.
How to Finalize your Application:
- Download and complete the application form (attached to the email and available below).
- Gather and submit the following required attachments:
- A copy of your Driver’s License or Military ID
- Include one of the following:
– DD-214 (long form including boxes 23–29)
– Active duty orders or HR verification letter
- Email your completed form and documents to: contact@hbot4heroes.org
Program Timeline
To remain eligible for funding, treatment must be completed within 4 weeks to 4 months of program approval.
