To finish your application:
- Fill out and return the attached document.
- Provide a copy of your DD214 or, if active duty your Military ID
- Submit documentation to Mills Lebo at mills.lebo@extivita.org
The Program Includes:
- A consultation with our Nurse Practitioner
- ANAM testing before and after treatment
- A course of 40 HBOT treatments
- Pro-Immune Supplement
- Two 30-minute consultation’s during treatment
Once your application is processed, we will be in touch with you regarding the next steps. Please note this program must be completed within 4 weeks-4 months in order to maintain your eligibility for funding.
