Hyperbaric Oxygen Therapy (HBOT) is helping veterans reclaim their clarity, focus, and hope. If you believe someone you care about could benefit, you can nominate them today. We’ll start by reaching out to you, not them.

State-Recognized Healing

Who Should Use This Form?

This form is for friends, family members, caretakers, neighbors and battle buddies – anyone who sees a veteran they care about struggling and wants to help them heal.

You might want to nominate someone if:

  • They’ve served in the military and show signs of TBI, PTSD, or emotional distress
  • They’re withdrawing, isolating, or facing challenges that feel bigger than themselves
  • They won’t take the first step on their own — but you know they need a lifeline

You can be that lifeline.

How the Nomination Works

We keep things simple and respectful — especially when it comes to privacy.

  1. You fill out the form below.
    Share your contact info and a few details about the veteran.
  2. We contact you first.
    Our care team will reach out to learn more and talk about the best way to approach the veteran. You’re not alone in this.
  3. If the time is right, we connect with them.
    If it makes sense and the time is right, we’ll connect with the veteran privately to offer support and explain the program.

Your referral stays confidential, and our team will always prioritize the veteran’s comfort, autonomy, and privacy.

Want to give them a heads up?

You’re welcome to let the veteran know you’ve submitted a referral — but you don’t have to. We’ll start by reaching out to you, and take it from there with care and discretion.
